Ratner Companies strives to provide a valued competitive benefits program for all our Associates. Benefits offered by Ratner Companies allows you to participate (at minimal, or, in some cases, no cost to you) in plans which protect you from – or help us treat – ailments, injuries and disease.

	Century Health Care (CHC):	Your health benefits are covered by CHC, a health care administrator who will quickly and accurately process your claims and submit payments to your providers. CHC will process your claims for inpatient services, outpatient exams, lab & x-ray and Rx requests with no deductible! No pre-existing condition limits! And convenient prescription drug coverage. After a six month wait, all Salon Associates are entitled to this valuable benefit (with the exception of “On-Call” Associates). If you average at least 25 hours per week, this benefit is fully paid by the company. The 401(k) plan is available immediately upon hire, to all Associates who are a minimum of 18 years of age. Choose any amount up to 50% of your salary – earn an additional $.33 for every dollar you contribute in a Company Match, up to $1,000 each year! The Company Match will begin after you have worked a minimum of six continuous months of service. Easy pre-tax deductions, full range of investment choices, and convenient access to your funds through the loan option. Prerequisite: Absolute Color (Level II). 	Eligibility:
	401(k) Retirement Savings Plan:

The following benefits are available to Associates who work an average of 25 hours per week or more:

	Dental:	Choose the Core Dental Plan or the Buy-Up Dental Plan. Both plans offer the same benefits with two differences: the Buy-Up plan will pay up to $1500 each year in dental expenses where the annual max on the Core plan is $1000, and if you elect the Buy-Up plan, your dependent children up to the age of 19 can receive Orthodontia benefits of $1500.
	Vision:	With a small co-pay, you can receive an exam, lenses, frames or contacts every 12 months. Please reference the coverage summary for details. We contract with independent Optometrists and Ophthalmologists, but you can visit any eye care professionals you wish.
	Supplemental Term Life Insurance:	Supplemental Life and/or Accidental Death & Dismemberment Insurance can be purchased during the annual Open Enrollment period for yourself and for your spouse or dependent children. Evidence of Insurability is required.

	Unum Voluntary Benefits:	If you choose to participate in the following benefits, you will actually own the policy and can continue participating even if you leave the employment of Ratner Companies. Ratner Companies offers the convenience of payroll deductions for all Unum benefits. Enrollment is limited to the annual Open Enrollment period.
	Unum Short Term Disability Insurance:	Disability Insurance can help replace a portion of your salary during a covered disability. Pre-existing limits apply, and as long as required premiums are paid when due, the policy is guaranteed renewable until your 72nd birthday.
	Unum Whole Life Insurance:	Provide added financial protection with this interest-sensitive policy in the event of your death, but it will also build cash value that you can utilize while you are still alive. The program also provides for additional benefits such as Spouse and Children Coverage, Waiver of Premium or an Accidental Death Benefit
	Unum Accident Insurance:	This plan offers a wide range of injuries and accident-related expenses such as hospitalization, intensive care, physical therapy and transportation. Choose to also cover your spouse and children. The plan also offers an optional rider for hospitalization due to sickness.
	Unum Critical Illness Insurance:	Critical Illness Insurance will pay a lump sum benefit to you upon your diagnosis – to use as you see fit. The plan pays upon first diagnosis for Heart Attack, Stroke, Cancer (enhanced rider), Coronary Artery Bypass, among others.

	LifeWorks Employee Assistance Program (EAP):	A CONFIDENTIAL resource available to you for counselors, guidance, and various help in the areas of parenting, elder care, financial management, legal concerns, stress, emotional well-being, addiction and recovery. Purchase computers and electronic equipment and accessories through Ratner’s partnership with Purchasing Power. All Associates with a minimum of 18 months of continuous service can take advantage of this program which is offered several times each year. Benefits include: no credit check; 12 month no-interest payment plan; convenient payroll deductions; brand-name computers and electronics; and convenient home delivery.
